34(1)The registrant shall, in the case of a court appointed receiver,
(a)
indicate the court that made the appointment,
(b)
indicate the judicial district, if any, of the court that made the appointment,
(c)
enter the court file number, and
(d)
enter, under the heading “Additional Information”,
(i)
the date of the order,
(ii)
the name of the applicant for the order, and
(iii)
the date of the appointment of the receiver.
34(2)The registrant shall, in the case of an appointment under a security agreement, enter, under the heading “Additional Information”, a statement indicating
(a)
the date of the appointment of the receiver, and
(b)
the name and address of the secured party named in the agreement.
