(a)
in nursing homes with thirty beds or more, the care of each resident is carried out by or under the supervision of a registered nurse as directed by the attending physician, or as directed by a nurse practitioner
(b)
in nursing homes with thirty beds or more, at least one registered nurse is on duty on the premises at all times,
(c)
in addition to the registered nurse referred to in paragraph (a), care staff is in attendance at all times in appropriate ratios, and
(d)
a comprehensive care plan is developed for each resident upon admission, reviewed at least annually and evaluated on an ongoing basis.
