REGULATION 86-99
under the
Mining Act

Filed June 27, 1986
  Under section 115 of the Mining Act, the Lieutenant-Governor in Council makes the following Regulation:
1This Regulation may be cited as the Forms Regulation Mining Act.
2In this Regulation
“Act” means the Mining Act.
3An oath or affirmation of confidentiality under section 20 of the Act shall be in Form 1.
4A prospecting licence under subsection 29(1) of the Act shall be in Form 2.
5A prospecting licence for a corporation under subsection 29(2) of the Act shall be in Form 3.
6A prospecting licence for a partnership under subsection 29(2) of the Act shall be in Form 4.
7An application to record a mineral claim under subsection 48(1) of the Act shall be in Form 5.
8A statement of work performed under subparagraph 56(1)(a)(i) or paragraph 71(1)(a) of the Act for an individual or partnership shall be in Form 6.
9A statement of work performed under subparagraph 56(1)(a)(i) or paragraph 71(1)(a) of the Act for a corporation shall be in Form 7.

11A statement verifying inspection and condition of boundary lines and claim posts under subsection 56(9) of the Act for an individual or partnership shall be in Form 9.
12A statement verifying inspection and condition of boundary lines and claim posts under subsection 56(9) of the Act for a corporation shall be in Form 10.
13A notice of dispute under subsection 61(1) of the Act shall be in Form 11.

16A mining lease under subsection 69(1) of the Act shall be in Form 14.
17A statement of information regarding interest holdings in a mining lease under subsection 79(1) of the Act for an individual or corporation shall be in Form 15.
18A statement of information regarding interest holdings in a mining lease under subsection 79(1) of the Act for a corporation shall be in Form 16.
19A transfer under section 101 of the Act shall be in Form 17.
20A notice of planned work on private land under paragraph 109(1)(a) of the Act shall be in Form 18.

21A certificate of non-payment under subsection 112(1) of the Act shall be in Form 19.
22A summary of work performed and statement of costs under paragraph 18(d) of the General Regulation - Mining Act shall be in Form 20.
