30The chief executive officer of a regional health authority, or a person designated by the chief executive officer, shall report, in accordance with the regulations, to a medical officer of health or a person designated by the Minister if an entry in the records of a hospital facility operated by the regional health authority states
(a)
that a person who is a patient in or an out-patient of the hospital facility
(i)
has or may have a notifiable disease or is or may be infected with an agent of a notifiable disease,
(ii)
has or may be affected by an injury or a risk factor prescribed by regulation, or
(iii)
has suffered a notifiable event prescribed by regulation; or
(b)
that a deceased person who had been a patient in or an out-patient of the hospital facility before that person’s death
(i)
had or may have had a notifiable disease or was or may have been infected with an agent of a notifiable disease,
(ii)
had been or may have been affected by an injury or a risk factor prescribed by regulation, or
(iii)
had suffered a notifiable event prescribed by regulation.
Reporting of notifiable diseases and other information
30The chief executive officer, or a person designated by the chief executive officer, of a regional health authority shall report, in accordance with the regulations, to a medical officer of health or a person designated by the Minister if an entry in the records of a hospital facility operated by the regional health authority states that a person who is a patient in or an out-patient of the hospital facility
(a)
has or may have a notifiable disease or is or may be infected by an agent of a communicable disease,
(b)
has or may be affected by an injury or risk factor prescribed by regulation, or
(c)
has suffered a reportable event prescribed by regulation.
