Under section 146 of the Provincial Offences Procedure Act, the Lieutenant-Governor in Council, on the recommendation of the Attorney General, makes the following Regulation:
1This Regulation may be cited as the Forms Regulation - Provincial Offences Procedure Act.
2In this Regulation
“Act” means the Provincial Offences Procedure Act.(Loi)
3An Information under section 2 of the Act shall be in Form 1.
4An Appearance Notice under subsection 5(1) of the Act shall be in Form 2.
5A Summons to Defendant under subparagraph 6(2)(a)(i) of the Act shall be in Form 3.
6A Warrant For Arrest of Defendant under subparagraph 6(2)(a)(ii) or paragraph 29(2)(b), 62(3)(b) or 73(4)(b) of the Act shall be in Form 4.
7A Notice of Cancellation of Appearance Notice under paragraph 6(3)(b) of the Act shall be in Form 5.
8A Certificate of Service of Appearance Notice under subsection 7(5) of the Act shall be in Form 6.
9A Plea of Guilty Form served with a Summons under subsection 8(1) of the Act shall be in Form 7.
10A Plea of Guilty Form served with an Appearance Notice under subsection 8(1) of the Act shall be in Form 8.
11A Notice of Return of Plea of Guilty Form under subsection 8(6) of the Act shall be in Form 9.
12A Ticket under section 9 of the Act shall be in Form 10.
13A Notice of Prosecution under subsection 11(3) of the Act shall be in Form 11.
14A Notice of Application for Leave to Dispute Charge under subsection 15(1) of the Act shall be in Form 12.
15A Certificate of Service under subsection 16(2) of the Act shall be in Form 13.
15.1A violation ticket under section 16.2 of the Act and the certificate referred to in paragraph 16.3(1)(j) of the Act shall be in Form 13.1.

16If a judge adjourns proceedings under section 27 of the Act so that a defendant may consider a change of plea, a Notice to Defendant in Form 14 may be sent to the defendant.
