Under sections 2, 126 and 173 of the Companies Act, the Lieutenant-Governor in Council makes the following Regulation:

1This Regulation may be cited as the Forms Regulation - Companies Act.
2In this Regulation, “Act” means the Companies Act.
3The application for incorporation required under section 7 of the Act shall be in Form 36.

5The application for change of name required under section 33 of the Act shall be in Form 45.
6Notice of the change of head office required under section 39 of the Act shall be in Form 46.
7Notice of the change of directors required under section 87 of the Act shall be in Form 47.
8Notice of the granting of supplementary letters patent required under sections 46 and 67 of the Act shall be as follows:

9The statement required under section 126 of the Act shall be in Form 35.
9.1Notwithstanding section 9, where the anniversary month of a company occurs after December, 1991, the statement required under section 126 of the Act shall be in Form 35.1.

9.2(1)In this section, “current annual return” means an annual return filed under section 126 of the Act for the current calendar year.
9.2(2)Where a company files a current annual return during the years 2003 or 2004, the obligation to file any and all past annual returns that were not filed is waived.
9.2(3)Where the anniversary month of a company falls within the last three months of the calendar year, the company may file an annual return for the year 2004 by April 30, 2005 and benefit from the waiver in subsection (2).
