1Section 23 of New Brunswick Regulation 94-33 under the Mental Health Act is amended
(a) in subsection (2) by striking out “Form 2” and substituting “Form 2.1”;
(b) in subsection (3) by striking out “Form 3” and substituting “Form 2.1”;
(c) in subsection (4) by striking out “Form 4” and substituting “Form 4.1”;
(d) in subsection (5) by striking out “Form 5” and substituting “Form 4.1”.
2Section 24 of the Regulation is repealed.
3Form 1 of the Regulation is amended
(a) by adding a text box in the top right hand corner with the following:
Person’s date of birth:
Medicare number:
(b) by striking out “on the ___ day of _______, 19___” and substituting “on the ___ day of _______, 20___”;
(c) by striking out “Signed this ___ day of _______, 19___” and substituting “Signed this ___ day of _______, 20___ at ____ hours”.
4Form 2 of the Regulation is repealed.
5The Regulation is amended by adding the attached Form 2.1.
6Form 3 of the Regulation is repealed.
7Form 4 of the Regulation is repealed.
8The Regulation is amended by adding the attached Form 4.1.
9Form 5 of the Regulation is repealed.
10Form 34 of the Regulation is repealed.
