Right to examine or copy personal health information
7(1)Subject to this Act, an individual has a right, on request, to examine or receive a copy of his or her personal health information maintained by a custodian.
7(2)A request made under this section shall
(a)
be made to the custodian that the individual believes has custody and control of the personal health information, and
(b)
contain sufficient detail to permit the custodian to identify and locate the record with reasonable efforts.
7(3)A custodian may require a request to be in writing.
