136The Chief Electoral Officer shall register in the Registry of Independent Candidates the name of any individual who files with him an application for registration in writing signed by the individual and setting out:
(a)
the full name and address of the individual;
(b)
the name of the electoral district in which he intends to be an independent candidate; and
(c)
the address to which communications intended for him may be addressed and where his books, records and accounts pertaining to contributions to and expenditures by him are or will be maintained.
