5(1)An application for a pet establishment licence or for the renewal of a pet establishment licence shall be made to the society on a form provided by the society.
5(2)An application or a renewal shall
(a)
include proof of compliance with zoning by-laws or zoning regulations made under the Community Planning Act, if a new premises is chosen,
(b)
include the name and address of the veterinarian who will provide his or her services to the pet establishment, and
